  • I hereby allow my child/ward to attend KIDS CAMP 2019 on the 6 and 7 December. I will not hold Ang Mo Kio Presbyterian Church responsible for any loss of property or damage to property suffered during the camp and any personal injury or death to my child/ward that may arise from his/her participation in the camp, provided reasonable care was taken by the church.
  • I hereby consent to the collection and use of the above given personal information for the purposes of the registration process for KIDS CAMP 2019. I also consent to receive information and updates from Ang Mo Kio Presbyterian Church. I give my consent that videos and photographs that are taken during the camp may be used for publicity, internal reporting and archival purposes.
